These Medicare Advantage plans come with health resources and benefits beyond what Original Medicare provides. These offerings include:

  • A $0 copay for an annual wellness visit.
  • National Access Plus, which allows you to see any doctor who accepts Medicare and our plans. You’re not tied to a provider network and you pay the same copay or coinsurance percentage whether your provider is in or out of the plan’s network.
  • Access to discounts and special offers from our healthy living partners.
  • A $0 copay for Select Generics prescription drugs (for Premier and Value plans that include Rx).
  • Prescription drug benefits with an extensive covered drug list (for Premier and Value plans that include Rx).
  • A broad nationwide pharmacy network (for Premier and Value plans that include Rx).

Note: These plans are entirely voluntary. Members will be responsible for their monthly premiums.

To enroll in a plan, download your enrollment guide and mail the completed enrollment form to the address noted.
